New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address, we do not take M23 area or some areas of M22.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor, if you do not have any identification for registration, that's fine, we do not need any to register you at our practice.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

YOUR DATA MATTERS

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.  You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.  NHS Digital will never sell your data.  There are strict rules about how NHS can use your data. It's only shared securely and safely.  Shared data helps the NHS. It has been used to find the first treatment for coronavirus and for vaccine research.

If you're happy with your confidential patient information being used for research and planning you do not need to do anything.
